What They Are
SNPs are Medicare Advantage plans tailored for specific needs — dual eligibility (Medicare + Medicaid), certain chronic conditions, or a nursing-home level of care. Most include Part D.
Types of SNPs
- D-SNP: For Medicare + Medicaid; often lower cost-sharing and extra benefits
- C-SNP: For qualifying chronic conditions (e.g., diabetes, CHF, COPD); condition-focused formularies and networks
- I-SNP: For residents in facilities or those meeting institutional level of care
